What is a Sprain and Strain?

Home Glossary What is a Sprain and Strain?

A sprain is a stretch or tear of ligaments supporting a joint, whereas a strain is an injury to muscle fibers or tendons from excessive stretching or forceful contraction.

Differentiating Sprains vs. Strains

A sprain involves tensile disruption of ligaments—the static fibrous bands connecting bones. Commonest in the lateral ankle (ATFL). Conversely, a strain involves microtrauma or mechanical tearing of contractile muscle fibers or their myotendinous junction (commonly hamstrings or calves).

Mechanism of Failure

Sprains occur when a joint is forced past its anatomical physiological range of motion (e.g., inversion twisting). Strains occur during maximal eccentric contraction where muscle fibers are stretched while attempting to shorten.

Classic Symptoms

  • Sharp sudden pain at the moment of mechanical failure
  • Rapid periarticular edema and progressive ecchymosis
  • Inability to bear full weight or severe pain during active muscle recruitment
  • Joint apprehension and subjective instability in high-grade sprains

Frequently Asked Questions

When is an X-ray mandated after an ankle sprain?

According to Ottawa Ankle Rules, radiographs are warranted if there is malleolar bone tenderness or inability to bear weight for four consecutive steps.

What is the typical return to physical activity?

Grade 1 injuries resolve in 1-2 weeks, while Grade 3 complete ruptures require 6 to 8 weeks of functional immobilization and proprioceptive neuromuscular retraining.
