What is a Peroneal Tendon Injury?

Home Glossary What is a Peroneal Tendon Injury?

A peroneal tendon injury is irritation, tearing or displacement of the tendons that run along the outer ankle and turn the foot outward. Repeated sprains and sudden changes of direction are common causes.

What Causes a Peroneal Tendon Injury?

The peroneal tendons on the outside of the ankle control side-to-side rolling during walking and running. Repeated ankle rolls, running on sloped ground and abrupt turns can make them rub inside their sheaths. If the tissue behind the outer ankle bone is weak, the tendons may slip over the bone.

Symptoms

  • Pain behind the outer ankle bone that increases with running
  • Tenderness and weakness when turning the foot outward
  • A snapping sensation as the tendons move over the bone
  • Swelling or burning along the outer ankle after prolonged activity

Frequently Asked Questions

Can a peroneal tendon problem be mistaken for a sprain?

Yes. Persistent outer ankle pain after a sprain deserves a tendon assessment. Examination, ultrasound and MRI can show tendon continuity and displacement. Rest, support, gradual exercise and balance training are common first steps; persistent slipping or a complete tear may require surgery.
