What is Sesamoiditis?

Home Glossary What is Sesamoiditis?

Sesamoiditis is irritation of the small sesamoid bones and nearby tissues beneath the big toe from repeated loading. Running, dancing and stiff or thin-soled shoes can increase pressure under the front of the foot.

What Causes Sesamoiditis?

The sesamoid bones beneath the big toe help the foot push off with every step, so repeated forefoot pressure can irritate them. A rapid increase in running, ballet and dance, high-heeled or thin-soled shoes, altered load distribution and a direct blow are common causes.

Symptoms

  • Pinpoint pain under the big toe at the front of the sole
  • Ache that increases when rising onto the toes or bending the big toe
  • Marked tenderness when pressing over the sesamoid bones
  • Mild forefoot swelling and discomfort inside shoes

Frequently Asked Questions

How is sesamoiditis distinguished from a sesamoid fracture?

Both can cause pain beneath the big toe. Examination considers the exact tenderness and onset; X-rays may show a fracture or shape difference, while MRI can show bone marrow swelling and nearby tissue changes. Reducing load, using a protective insole and choosing suitable shoes are common first steps.
