What is an Acromioclavicular Joint Separation?

Home Glossary What is an Acromioclavicular Joint Separation?

An acromioclavicular joint separation is stretching or tearing of the ligaments joining the collarbone to the acromion after a fall onto the shoulder or a direct blow. The outer end of the collarbone may move upward.

What Causes an Acromioclavicular Joint Separation?

Landing on the outer shoulder during cycling or contact sports suddenly loads the acromioclavicular and coracoclavicular ligaments. A mild injury stretches them, while a more severe tear can leave the outer collarbone visibly higher. Examination and shoulder X-rays help determine the grade.

Symptoms

  • Sharp pain on top of the shoulder near the outer collarbone
  • Swelling or a step-like prominence at the shoulder tip
  • Pain when lifting the arm overhead or bringing it across the chest
  • Tenderness that worsens when lying on the shoulder or wearing a bag

Frequently Asked Questions

Does an acromioclavicular separation always need surgery?

No. Low and moderate grade injuries can recover with a sling, pain control and gradual motion. Surgery may be discussed when there is marked displacement, persistent pain, weakness or a high demand for overhead work and sport, based on examination and imaging.
