Severe shoulder pain is a symptom that must be taken seriously, as it can arise from various causes ranging from musculoskeletal issues to internal organ disorders. Shoulder pain that starts suddenly, restricts daily movements, or wakes you from sleep at night particularly suggests that the underlying problem may go beyond a simple strain. Therefore, accurately evaluating the potential causes of severe shoulder pain is of great importance.

Musculoskeletal and Joint Causes of Severe Shoulder Pain

Shoulder impingement and tendon problems

Shoulder impingement syndrome and damage to rotator cuff tendons can cause pain that increases when lifting the arm and worsens over time. In these conditions, loss of strength and restricted range of motion in the shoulder are common.

Frozen shoulder (adhesive capsulitis)

Frozen shoulder develops with stiffening of the joint capsule and can manifest as sharp, constant shoulder pain that particularly intensifies at night. Restricted mobility becomes noticeable.

Trauma and strains

Shoulder injuries occurring after falls, direct impacts, or sudden overloading can cause severe and acute pain. Fractures or serious tears are evaluated within this group.

Internal Organ Causes of Severe Shoulder Pain

Heart-related shoulder pain

Pain of cardiac origin can present as a sensation of pressure or squeezing, particularly radiating to the left shoulder and arm, and may be life-threatening.

Gallbladder and digestive system problems

Gallbladder issues can often cause severe pain that radiates to the right shoulder and shoulder blade region.

Lung and chest-related problems

Problems involving the pleura or certain infections can cause pain that increases with breathing and radiates to the shoulder.

When Should Severe Shoulder Pain Be Taken Seriously?

Shoulder pain that wakes you from sleep at night

Pain that increases at night may be a sign of frozen shoulder or serious tendon damage and should definitely be evaluated.

Inability to lift the arm and loss of strength

If shoulder pain is accompanied by an inability to raise the arm or sudden weakness, prompt specialist evaluation is necessary.

Conditions requiring emergency intervention

If shoulder pain is accompanied by shortness of breath, chest pain, sweating, dizziness, or nausea, emergency medical care must be sought immediately.
